SUMMARY:

Provide direct support to people with developmental disabilities or related conditions by serving as an advocate, guide, teacher, and companion in accordance with Person Centered Plans. Support the agency mission to provide individualized community life experiences to individuals with developmental disabilities and brain injuries.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

Direct Care

  • Implement each persons’ Individual Person Centered Plans, and Coordinated Services and Support Plans (CSSP), which may include assisting both males and females with all personal cares, including appropriate dress and grooming.
  • Follow the Individual Abuse Prevention Plan as written for each person.
  • Follow the Health Needs record as written for each person, administer medication as scheduled, apply prescribed treatments and complete chart accurately and timely as directed.
  • Implement and follow through with positive behavior supports and document all appropriate information.
  • Work with each individual supported on his or her person centered objectives.
  • Support each persons’ activities of his or her choice at home, in the community and out of town. Activities may include swimming, going to church, shopping, driving, etc.
  • Provide information, opportunities, and support to develop individual interests and relationships of each person.
  • Communicate necessary information regarding each person to appropriate individuals and provide documentation as required.
  • Provide a clean, safe, and comfortable environment for each person by following the list of cleaning duties and safety procedures.
  • Prepare and provide a variety of foods according to meal plans, which may include pork and pork products, special dietary needs, or individual client preferences.

Administrative

  • Purchasing household needs and organizing.
  • Communicate any maintenance or household needs to appropriate staff in a timely manner.
  • Ensure that at the start of each shift the appropriate charts, logs, and reports are read for updates. Chart in the electronic communication system, Therap, the appropriate updates at the end of each shift, ensure its accuracy and appropriateness, and relay necessary information to other staff. Read all SComm communications to ensure understanding of upcoming appointments, changes, and other necessary information.
  • Check charting on therap (outcomes, medications, checklists) and report to Program Manager.
  • Give weekly/monthly updates to Program Manager as requested.
  • Assist individuals in setting up and ordering medications.
  • Complete assessments on individuals as requested by Program Manager.
  • Complete monthly financial spreadsheets.
  • Assist, train and coach individuals to pay their monthly bills. These bills may include rent, utilities, cell phones, etc.
  • Serve as trainer, coach, and mentor to new and other staff members. Assist the Program Manager in completing all training documentation required by 245D.
  • Function as lead staff in the absence of the Program Manager.
  • Perform medication check at the start and end of each shift as needed.
  • Accurately record hours worked through the electronic time clock system.
  • Attend and participate in required meetings and/or trainings.
  • Maintain a professional and safe work environment and adhere to all organizational and department policies and procedures, specific safety standards and guidelines.
  • Support the agency marketing, development, and fundraising events through participation, communication, or other means of support.

MINIMUM QUALIFICATIONS:

Education and/or Experience

 

High School diploma or GED. Six months’ experience working with individuals with developmental disabilities, brain injuries, or closely related field.

 

Must be 18 years of age or older.

 

Must successfully pass the criminal background check.

 

Must be willing to work with both males and females.

 

Must have a valid Minnesota driver’s license and proof of insurance.

 

KNOWLEDGE, SKILLS, AND ABILITIES:

 

Knowledge and experience in working with people with developmental disabilities.

 

Skill in establishing and maintaining effective working relationships with clients, families, and staff.

 

Sensitivity in dealing with clients, their families or guardians.

 

Good oral and written communication skills.

 

Ability to work collaboratively and support efforts of team members.

 

Ability to work with individuals with developmental disabilities, brain injuries, or other related disabilities.

 

Ability to understand English and follow oral and written instructions.

 

Ability to communicate clearly with clients, families, and staff.

 

Ability to manage time effectively.

 

Ability to manage work with flexible schedules.

 

Ability to maintain confidential information.

 

Excellent sensory skills to include hearing, vision, and speech.

 

Maintain a commitment to the agency’s mission, vision, and values.

While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to stand and walk. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl. The employee must frequently lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

 

WORK ENVIRONMENT:

While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.,
